Housing Stabilization Services (HSS)
Housing Stabilization Services is a Minnesota Medical Assistance program to help people with disabilities, including mental illness and substance use disorder, and seniors find and sustain housing.

The 3 Components of Housing Stabilization Services
Housing Consultation Service
- A service to develop a person-centered plan for people without MA case management services.
- Developing a Housing Focused Person-Centered Plan based on assessment outcomes.
- Gathering all required documentation and identifying needs based on strengths, needs and cultural preferences.
Housing Transitioning Service
- A service that supports a person to find housing.
- Moving Expenses – may be available to a person receiving Housing Stabilization-Transition services who are transitioning out of a Medicaid-funded institution or leaving a provider-operated living arrangement and moving into their own home.
- We help with with housing search and applying, tenant screening, personal financial budgeting, navigating transportation challenges, building relationships with landlords, and securing a lease.
Housing Sustaining Service
A service that supports a person to maintain housing. This helps ensure that individuals remain in stable housing and avoid the risk of homelessness.
