Housing Stabilization Services is a Minnesota Medical Assistance program to help people with disabilities, including mental illness and substance use disorder, and seniors find and sustain housing. There are three services included:

• Housing Consultation Service  
• Housing Transitioning Service 
• Housing Sustaining Service

Specialized Equipment and Supplies

Devices (including assistive technology), controls, medical appliances or supplies specified in the person’s support plan.
